Publisher's binding of white vellum with an Arts and Crafts rose and the words "Soul is Form," housed in a later slipcase. 7 1/4 x 4 3/4 inches (18 x 12 cm); hand-colored frontispiece woodcut by Reginald Savage, 21, [3] pp., with initials in gold and colors, printed in black and red. Slight cover soiling, with William T. Shirley Jr.'s bookplate on the front endpaper.
